The military record requested the veteran to list name, present address, residence at time of entering service, when and where subject entered service, discharge information (date, place, reason for discharge, rank/role at discharge), military organizations subject was a part of, appointments/promotions/commissions received, where stationed, and prior military service (before war). It also asked what battles/major operations subject took part in, war injuries, hospitalizations, citations received, and, for overseas service, U.S. departure and return information. The personal information requested included full name, birth information, schooling, occupations before and after discharge, parents' names and birthplaces, siblings' names and military service, marriage information (date, place, spouse, and her parents), children and ages, and ancestors or uncles who served in the Army or Navy. There was also space for the veteran to share any of his Army or Navy experiences that he felt should be preserved.
