This is an evaluation form for Willard S. Richardson for attendance to the School for Christian Workers (now Springfield College) in their YMCA Secretarial training school department. The name of the person who filled out the form is hard to make out, but "Latze" may be the last name. The form is dated as having been filled out on August 28, 1889. The document asks questions about character, experience, religion, health, and other questions the school deemed important to the decision of whether or not to accept him as a student.

Richardson was born in Painesville, Ohio and attended Denison University for two years. He also studied at the University of Rochester, worked at Springfield College, worked at Columbia University, and received his B.D. from the Union Theological Seminary in 1897. From 1894-1909, he worked as the assistant pastor at a Baptist church in New York City. He later joined the advisory staff to John D. Rockefeller, Jr., and was secretary for the Laura Spelman Rockefeller Memorial in New York City. Richardson served on the Board of Trustees at Springfield College and the American Institute of Christian Philosophy. In 1938, Springfield College presented him the Tarbell Medallion, an award for alumni who have demonstrated varied outstanding service to their alma mater.
